Service 1: Music Therapy for people receiving hospice or palliative care
Our goal is to provide dignity, emotional comfort, spiritual support, pain alleviation and life reminiscence through music therapy during care for a life limiting illness. The vision is compassionately giving people and their loved ones the music that eases distress and celebrates their lives. In this process there is value in leaving your troubles with the music, genuine interactions with everyone served, smiles and tears, generous giving of time through assessment to develop solutions for each patient including visiting friends and family, music that is meaningful to the patient, active listening and reflection, and cultural sensitivity to learn about and honor family values, traditions and faiths. Services may be requested or referred by patient, primary caregiver or interdisciplinary group with consent/approval of patient.
How? Patient, primary caregiver or interdisciplinary group member contacts HOPEFORTH. Within 48 hours, HOPEFORTH will contact the person who requested services to schedule the first visit assessment. First visit assessment is 20 minutes to an hour depending upon conversation and needs assessed.
Why? Hope. When someone is diagnosed with a terminal illness, hope is ever-changing. When an element of hope is discovered, things change. HOPEFORTH's impetus is to provide music therapy for all of these changes in hope through emotional comfort, spiritual support, symptom alleviation, self-care, and life review. We want to bring HOPEFORTH for you.
